Minivis anchorage and molar protraction
Congenital absence of a lower 2nd premolar.
At the start of treatment, the lower primary 2nd molar shows no significant infraclusion.
During the course of treatment, progressive infraclusion of the primary tooth was observed, confirming ankylosis.
We decided to proceed with extraction and protraction of the 1st and 2nd lower molars.
An 8 mm anchorage miniscrew was inserted between the canine and the 1st premolar.
A cantilever bracket inserted distally on the 1st molar applied force below the molar’s center of resistance.
Progression shows parallel protraction of the molar, enabling a definitive solution without the need for an implant in position 35. Note that the impacted 3rd molar has migrated mesially with the 1st and 2nd molars.
