{"id":22247,"date":"2013-04-05T00:00:00","date_gmt":"2013-04-05T00:00:00","guid":{"rendered":"https:\/\/sylvainchamberland.com\/idiopathic-condylar-resorption\/"},"modified":"2023-08-21T14:16:32","modified_gmt":"2023-08-21T14:16:32","slug":"idiopathic-condylar-resorption","status":"publish","type":"post","link":"https:\/\/www.sylvainchamberland.com\/en\/idiopathic-condylar-resorption\/","title":{"rendered":"Idiopathic condylar resorption"},"content":{"rendered":"<h3>Case study<\/h3>\n<p>A 22-year-old man underwent MRI and CBCT examinations which show the resorption of his right condyle. He had pain in the right jaw joint about one year and a half ago and the pain intensified about 6 months ago. This young man had an orthodontic treatment when he was 13 years old and a joint noise was noticed at that time.<\/p>\n<p>&nbsp;<\/p>\n<p>To learn about the whole story, follow this link: <a href=\"http:\/\/www.orthodontisteenligne.com\/dentition\/articulations-atm\/comment-page-1#comment-14216\" target=\"_blank\" rel=\"noopener\" class=\"broken_link\">click here<\/a>.<\/p>\n<p>&nbsp;<\/p>\n<p>Here is my point-by-point analysis.<\/p>\n<p>1\u2014 The joint noise observed at about 13 years of age was presumably the sign of a disc displacement with reduction. This is frequent in teenagers and does not foreshadow any consequences. The causes are multifactorial. It is not necessary to go into details in this column.<br \/>\n2\u2014 The pain felt a year and a half (1 \u00bd) ago is the important item to remember. It is possible that your son had what is called a <strong>disc displacement without reduction<\/strong> (the disc becomes stuck in front of the joint). See on YouTube: <a href=\"http:\/\/www.youtube.com\/watch?v=TnNgj1uUT6Y\" target=\"_blank\" rel=\"noopener\">TMJ anterior disc displacement without reduction<\/a>.<br \/>\nIt is possible that your son had <strong>problems opening his mouth<\/strong> and that it <strong>would deviate on the side where he felt pain<\/strong>. It was the beginning of a mechanism of chronic injury which caused<strong> inflammation in the joint<\/strong>.<\/p>\n<p>3\u2014 Inflammation changes the viscosity of synovial fluid which is designed to lubricate the joint. Researchers have shown the presence of <strong>free radicals<\/strong> when inflammation is present. <strong>Cytokines<\/strong> are secreted, they activate osteoblasts which, in turn, activate osteoclasts. <strong>Osteoclasts secrete proteinases<\/strong> (matrix metalloproteinases (MMPs)). They are <strong>endopeptides involved in degradation of the extracellular matrix of collagen and elastin<\/strong> of articular surfaces of the TMJ. This results in <strong>resorption<\/strong>.<\/p>\n<p>4\u2014 A person&#8217;s susceptibility for condylar resorption depends on several factors: <strong>gender<\/strong> (male, female), <strong>nutritional state<\/strong> (anorexia, bulimia), genetics (inheritance), <strong>perioral habits<\/strong> (nail or cuticle biting, teeth clenching, gum chewing) and <strong>iatrogenic compression<\/strong>.<\/p>\n<p>5\u2014 An article written by Gunson and <strong>Arnett<\/strong> and recently approved for publication discusses a <strong>pharmacological approach for the control of mandibular condylar resorption<\/strong>: cytokine blockers, control of MMPs, prostaglandin and leukotriene inhibitors, IL-6 receptor inhibitors.<br \/>\nResearch is thus very promising.<\/p>\n<p><a href=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/Attachement-du-disque-a-larriere-du-condyle-orthodontiste-Chamberland-Quebec.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-full wp-image-3630\" title=\"Disc-attachment-at-the-back-of-condyle-orthodontist-Chamberland-Quebec\" alt=\"\" src=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/Attachement-du-disque-a-larriere-du-condyle-orthodontiste-Chamberland-Quebec.jpg\" width=\"275\" height=\"196\"><\/a> <strong><\/strong><\/p>\n<p><strong>Wolford<\/strong> recommends the repositioning and <strong>ligature of the disc posterior to the condyle<\/strong>. Tissue in excess in the bilaminary zone (posterior to the disc) is cut and removed.<\/p>\n<p>The disc is mobilized and repositioned. Two Ethibond sutures are inserted through the eyelets of the Mitek anchor. The anchor is inserted into the posterior head of the condyle, 8 mm from the head, laterally from the center of the condyle. The sutures are attached to the posterior band of the disc.<\/p>\n<p>&nbsp;<\/p>\n<p>6\u2014 As far as your son is concerned, I recommend a <strong>complete blood count<\/strong>, the dosage of Rh factor and of surface antigens, <strong>sedimentation rate<\/strong>, testosterone level. I also recommend a <strong>Tc-99 scintigraphy<\/strong> to determine if there is a higher bone turnover on the right side.<br \/>\nWearing a plate is useful to reduce the parafunctions. Physiotherapy is indicated. Taking anti-inflammatory drugs is indicated (Celebrex, Feldene). Consult with a physician or a rheumatologist regarding this.<br \/>\nNew X-rays will have to be taken in 6 months and in 1 year and a CBCT in 1 year.<br \/>\n7- In short, your son has arthrosis on the right TMJ. It is confirmed, for that matter, by the examinations he had.
