You have an excellent question that is worth thinking about.<\/p>\n<p><span style=\"font-size: medium;\"><strong>Can we live without canines?<\/strong><\/span><\/p>\n<p>The answer is <span style=\"font-size: large;\"><strong>yes<\/strong><\/span>, we can live without canines. Except that you are <span style=\"font-size: medium;\"><strong>not asking the right question<\/strong><\/span>.<\/p>\n<p>You mention having to undergo a surgery which would be done to expose and ligature the impacted canine for an orthodontic traction. Yet, you <strong>refuse<\/strong> the surgery. This means that <strong>your canine will stay impacted<\/strong>.<\/p>\n<h3>The right question<\/h3>\n<p>The right question to ask is:<\/p>\n<p><span style=\"font-size: medium;\"><strong>What are the risks associated with keeping one or more impacted canines?<\/strong><\/span><\/p>\n<p>First, I will show you a few examples of occlusion without permanent canines. Then I will illustrate a few examples of damage that an impacted canine can cause.<\/p>\n<h2>Occlusion without canine(s)<\/h2>\n<div id=\"attachment_6733\" style=\"width: 810px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/occlusion-sans-canines-Chamberland-Orthodontiste-Quebec.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-6733\" class=\"size-full wp-image-6733\" title=\"Occlusion without canines\" src=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/occlusion-sans-canines-Chamberland-Orthodontiste-Quebec.jpg\" alt=\"occlusion-without-canines-Chamberland-orthodontist-Quebec\" width=\"800\" height=\"135\" \/><\/a><p id=\"caption-attachment-6733\" class=\"wp-caption-text\">Case #1 and case #2: canine #13 was impacted and ankylosed. Case #3: Agenesis of 2 permanent canines #13 and #23.<\/p><\/div>\n<p style=\"text-align: left;\">Cases #1 and #2 each had their canine #13 impacted and ankylosed, that is fused to the bone. It was thus impossible to proceed with their orthodontic traction. The removal of the impacted tooth was necessary. The patients were told that the removal of 3 premolars (#24, #34 and #44 for case #1 and #24, #35 and #45 for case #2) would be necessary to obtain an ideal occlusion. The Xs represent the spot where the canines would be if they existed. The first premolars #14 and #24 are identified. The replacement of canines by first premolars allows the patient to obtain an acceptable esthetic and functional result.<\/p>\n<div id=\"attachment_6737\" style=\"width: 292px\" class=\"wp-caption alignleft\"><a href=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/ankylose-de-canine-incluse-_13_Chamberland-Orthodontiste-Quebec.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-6737\" class=\"size-full wp-image-6737\" src=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/ankylose-de-canine-incluse-_13_Chamberland-Orthodontiste-Quebec.jpg\" alt=\"ankylosis-of-impacted-canine-_13_Chamberland-orthodontist-Quebec\" width=\"282\" height=\"266\" \/><\/a><p id=\"caption-attachment-6737\" class=\"wp-caption-text\">Ankylosed canine<\/p><\/div>\n<p style=\"text-align: left;\">The picture on the left represents the <strong>ankylosed canine<\/strong> of case #2. The tooth crown is delimited by the dotted line. No space was visible between the enamel and the bone, confirming the ankylosis (fusion to the bone) of the tooth. The removal was the only possible option.<\/p>\n<p>Case #3 presented agenesis of the permanent upper canines (he never had them). Two premolars had to be extracted in the mandibular arch.<\/p>\n<p>These cases thus show that it is possible to live without canines and have a functional occlusion.<\/p>\n<h2>Risks associated with an impacted canine<\/h2>\n<p>You must know that if you do not do anything and you keep your impacted canine, you are taking risks.<\/p>\n<h3>1- Formation of a cyst of the follicular envelope<\/h3>\n<div id=\"attachment_6739\" style=\"width: 302px\" class=\"wp-caption alignleft\"><a href=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/Lesion-apical-canine-primaire-et-kyste-folliculaire-canine-incluse-Chamberland-Orthodontiste-Quebec.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-6739\" class=\"size-full wp-image-6739\" title=\"Dentigerous or follicular cyst\" src=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/Lesion-apical-canine-primaire-et-kyste-folliculaire-canine-incluse-Chamberland-Orthodontiste-Quebec.jpg\" alt=\"Apical-lesion-primary-canine-and-follicular-cyst-impacted-canine-Chamberland-orthodontist-Quebec\" width=\"292\" height=\"335\" \/><\/a><p id=\"caption-attachment-6739\" class=\"wp-caption-text\">Dentigerous or follicular cyst. The 13 indicates the permanent canine. The \u201cC\u201d indicates the primary canine. The dotted white line indicates the extent of the cyst.<\/p><\/div>\n<p>A chronic apical lesion of the primary canine facilitated the development of a follicular cyst and the exaggerated movement of the permanent tooth #13.<\/p>\n<p>This exaggerated upward and forward movement of the impacted canine makes the de-impaction treatment more difficult and longer.<\/p>\n<h3>2- Radicular resorption of permanent incisors<\/h3>\n<div id=\"attachment_6742\" style=\"width: 710px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/canine-incluse-bilaterale-et-resorption-radiculaire-JaSa-Chamberland-Orthodontiste-Quebec.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-6742\" class=\"size-full wp-image-6742\" src=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/canine-incluse-bilaterale-et-resorption-radiculaire-JaSa-Chamberland-Orthodontiste-Quebec.jpg\" alt=\"Bilateral-impacted-canine-and-radicular-resorption-JaSa-Chamberland-orthodontist-Quebec\" width=\"700\" height=\"288\" \/><\/a><p id=\"caption-attachment-6742\" class=\"wp-caption-text\">Resorption of permanent central incisors caused by two impacted canines<\/p><\/div>\n<p>Incisors can suffer from resorption of their root. Note that the central incisors lost at least 50% of their length because of the impacted canines. Figuratively, we could say that there was a collision between the canine and the root of the incisor and the root &#8220;disappeared&#8221;.<\/p>\n<p>In that case, we decided to extract the impacted canines, because it would have been impossible to perform the traction of these canines without causing more damage to the roots of the incisors.<\/p>\n<div id=\"attachment_6753\" style=\"width: 321px\" class=\"wp-caption alignright\"><a href=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/Canine-incluse-et-resorption-radiculaire-Chamberland-orthodontiste-Quebec.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-6753\" class=\"size-full wp-image-6753\" src=\"https:\/\/sylvainchamberland.com\/wp-content\/uploads\/2023\/04\/Canine-incluse-et-resorption-radiculaire-Chamberland-orthodontiste-Quebec.jpg\" alt=\"Resorption of permanent lateral incisors caused by impacted canines\" width=\"311\" height=\"215\" \/><\/a><p id=\"caption-attachment-6753\" class=\"wp-caption-text\">Resorption of permanent lateral incisors caused by the impacted canines<\/p><\/div>\n<p>This patient was lucky, because sometimes, one or more incisors need to be extracted like in this other case shown on the right.<\/p>\n<h3>3- Ankylosis of the impacted tooth<\/h3>\n<p>This problem was shown and discussed in the previous section. In such case, the removal of the ankylosed tooth is part of the solution.<\/p>\n<h2>Conclusion<\/h2>\n<p>This is just a brief summary of what can happen with an impacted canine. I did not describe every risk. It would be too long doing so.<\/p>\n<p>Going back to your case, I want to understand that you do not want any surgery. But you will have to <strong>assume the risks<\/strong> and complications which will <strong>inevitably<\/strong> occur because of your decision.<\/p>\n<p>Between two evils, you have to choose the lesser one.
